My name is Gary, and my wife and I own Guardian Grounds Homes, a supportive housing provider in Sacramento, California.
We’re looking to connect with additional referral partners and expand our network. We currently have availability and are interested in building long-term relationships with organizations that serve veterans, justice-involved individuals, people transitioning from homelessness, and others in need of stable, supportive housing.
I’m also looking to learn more about Medi-Cal/CalAIM Community Supports and which Managed Care Plans, hospitals, behavioral health providers, nonprofits, and community organizations have been your best referral sources.
If you’ve had success with specific organizations or have recommendations on who we should connect with, I’d greatly appreciate any advice or introductions.
